Job Description:

Objective:The Home Care Consultant/Client Care Coordinator (HCC) is responsible for ensuring achievement of revenue objectives for the assigned territory. The primary duty of the HCC is to build relationships with referral providers so they refer clients as they are discharged from their facility and to build relationships with other referral sources.-- Expected to perform a variety of duties that relate to client care including care consultations with potential clients and family members, client/caregiver introductions, quality assurance visits with existing clients, problem resolution and scheduling. Some travel and work outside of normal office hours in the evening and on weekends is required. The HCC must be able to collaborate and work as a team with other office employees.Primary Responsibilities:&bull Reflect the core values of Associated Services LLC (d.b.a. an independently owned and operated Home Instead franchise).&bull Responsible for researching and developing client relationships and producing referrals performs all phases of sales process, including preparation and documentation of cold calls, follow-up calls, diagnosis of referral source and client needs, recommendation of appropriate Home Instead-- services and additional community resources, maintaining and nurturing existing referral accounts, including follow-up, as necessary.&bull Conduct Client/CAREGiver introductions with every new client and with every new CAREGiver.--&bull Conduct ongoing quality assurance (QA) site and phone visits with clients according to schedule.&bull The HCC is the point person for the resolution of client and caregiver issues as they are escalated during the daily operations of the business.&bull Estimated time allocation for this position is:o Soliciting and onboarding new clients &ndash 40%o Marketing and building relationships with referral providers &ndash 10%o Quality Assurance evaluations - Case Management--&ndash 20%o Resolving escalated client/caregiver issues &ndash 10%o Scheduling activities &ndash 20%&bull Achieve inquiry generation objectives with the primary goal of bringing in new business to increase overall market share. Target accounts include, but are not limited to, hospitals, physicians, home health agencies, assisted living facilities, nursing homes, senior centers, hospice, funeral homes, etc.--&bull Travel to clients' homes or to facilities and meet with potential clients and family members to discuss their needs and provide solutions in the form of a service plan &ndash 10%-20% travel, as needed.&bull Conduct Service Inquiries and Care Consultations as needed following the consultative sales process.&bull In conjunction with Home Instead franchise leadership, develops sales and marketing plans, detailing weekly and monthly activities focused on meeting or exceeding established goals.&bull Develop and maintain knowledge of Home Instead brand.-- Effectively presents Home Instead marketing materials and services to referral providers, including 1:1 meetings, as well as planning, arranging and conducting formal group presentations to referral providers.--&bull Maintain accurate records of all sales and prospecting activities including sales calls presentations closed sales and follow-up activities within the assigned territory, including the use of software to maintain accurate records to maximize territory potential.--&bull Develop a database of qualified leads from referrals through face-to-face cold calling on referral providers, email, telephone and web.--&bull Adhere to all company policies, procedures and business ethics codes.--&bull Participate in and contribute to the development of educational programs offered to clients, caregivers, prospects, and co-workers.&bull Support all scheduling activities, as needed and other duties as assigned.&bull Collaborate with co-workers and assist in other job functions, as needed.Starting Salary $35-$40K depending upon experience. Vacation and Sick Pay after probationary period. No medical benefits.Each Home Instead franchise is independently owned and operated.
