Supportive housing - affordable housing paired with services designed to help people maintain their homes for the long-term - is widely recognized as a proven and cost-effective solution to chronic homelessness.But we didn't stop at providing housing. Breaking Ground's programs and services help people experiencing street homelessness - especially those who have been on the streets the longest - to come indoors. Through our Street to Home outreach program and transitional housing resources, we help people get, and stay, on the path to a permanent home.The Scatter Site program provides community-based, independent apartment housing located throughout New York City and case management services for clients who have experienced chronic homelessness, substance use, and mental health challenges.Reporting to the Director, Programs, Scatter Site, the Assistant Director, Programs, Scatter Site is responsible for supporting clinical operations for the Scatter Site program. The Assistant Director manages Clinical Coordinators and oversees the leasing and maintenance of apartments. The Assistant Director helps to maintain strong partnerships with internal and external partners including landlords, community leaders, outreach, and transitional housing staff. Utilizing Harm Reduction and Housing First strategies, the Assistant Director implements clinical best practices for the case management teams.The Assistant Director works primarily during business hours Monday-Friday, with off-hours availability in case of emergency or for special projects/initiatives.ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Supervises Clinical Coordinators and supports effective management of Case Managers
  • Provides regular, structured supervision meetings for direct reports
  • Promotes effective strategies for case management and placement of clients into permanent housing following Housing First, Harm Reduction, Crisis Prevention Institute (CPI), Motivational Interviewing, and trauma-informed care models of practice
  • Oversees development and implementation of individual service plans; ensures documentation of client contact and progress is comprehensive and timely
  • Supervises and conducts case conferences
  • Coordinates delivery of care with multiple service providers
  • Provides support and guidance to front-line staff in managing difficult situations and successfully de-escalating conflict
  • Provides guidance for incident management, crisis interventions, risk assessments, and ensures all documentation is submitted to appropriate oversight entities in a timely manner
  • Promotes a staff and resident culture that emphasizes housing placement as a measurement of success; ensures that tenants successfully retain their housing placement
  • With direction from the Director, responds and engages with external partners, government representatives, community stakeholders, hospital personnel, and other agencies to ensure responsive and effective service planning and program operations
  • Performs other duties as assignedMINIMUM QUALIFICATIONS:
    • Master's Degree in Social Work and an active LMSW license is required. Must obtain LMSW within the first six months of employment. LCSW preferred
    • Minimum 2 years of supervisory experience
    • Experience working in 24/7 programs and with homeless or formerly homeless populations strongly preferred
    • Thorough clinical understanding of relevant service delivery concepts and structures including Housing First, client-centered motivational interviewing, Harm Reduction models, behavioral and medical health systems, and the ability to access and negotiate the full range of services for recipients
    • Experience with crisis intervention, including risk assessments and incident management
    • Ability to work successfully with a wide range of internal and external stakeholders
    • Demonstrated success in working in a fast-paced environment with multiple priorities
    • Ability to delegate and motivate staff to achieve deliverables
    • Ability to communicate (verbally and written) with diverse populations and stakeholders
    • Proficiency with Microsoft Office Suite programs including Word, Excel, Outlook, and Microsoft Teams; familiarity with Access-based databases and the ability to learn new programs are required
