Our client, a reputable organization that drives solution to transform the lives of people with disabilities, people in poverty and people suffering from substance abuse/addition, has an immediate need for a LPN Behavioral HealthPosition Type: HC ContractLocation: Brooklyn, NYEssential Duties & Responsibilities

• Take vital signs, such as blood pressure, temperature and weight
• Assist with diabetes medication and treatment
• Take medical histories and enter information in computer systems
• Coordinate with Case Managers, Senior Case Managers and Director of Social Services to ensure proper care, nutrition and treatment plans are followed
• Follow up with community health care providers to ensure medical treatment plans are documented, monitored and followed
• Have flexibility to work a schedule that includes evenings and weekendsQualifications
  • New York State Licensed Practical Nurse.
  • Basic knowledge of human services
  • Bilingual English/Spanish preferred but not required.
  • Knowledge of chemical dependency as a primary diagnosis.
  • Excellent written and verbal communication
  • Assessment and case management
  • Knowledge of Microsoft Office tools
