- Provide care to assigned patients through assessment of patient
needs, developing and implementing an interdisciplinary plan of
care, which reflects an individualized mode of nursing and medical
intervention so that the appropriate outcome can be achieved in a
timely manner.
- Initiates a Nursing Assessment and interdisciplinary
treatmentdischarge plan upon admission; which reflects patient
teaching and discharge plan according to established guidelines.
- Allows patients and/or significant others, as much as possible
to collaborate in the plan of care. Develop a discharge plan in
collaboration with the physician, patients/family other disciplines
utilizing the available resources.
- Execute physician orders concerning medication and therapy and
evaluates patient response with a clear understanding of the
scientific principles of cause and effect.
- Collaborates with the Head Nurse in delegating assignments for
nursing personnel based on patients' acuity and employee skill
level.
- Records pertinent observations on patient chart and reports
findings to Head Nurse and/or physician.
- Evaluates the results of nursing intervention and
malesappropriate changes when necessary.
- Able to monitor suicidal patients and document observations
asper hospital policy.
- Administer medications and treatments in a safe knowledgeable
manner and document same according to hospital policy, procedure,
and professional standards.
- Prepare for and assist with diagnostic procedures and
treatment.
- Demonstrates proficiency in the insertion of peripheral
cannulation, and administration of nebulizer and/or metered dose
inhalation therapy.
- Count narcotics with on-coming and outgoing tour nurses
according to hospital policy/procedure.
- Check code cart/emergency equipment on a daily/tour by tour
basis-open according to service protocol.
- Counsel and/or instruct patients and/or families with evidence
of the instructions and the patients' or families' understanding of
the instructions documented in the progress note and teaching
tool.
- Evaluates interdisciplinary treatment discharge plan on an
ongoing basis and makes modification as indicated.
- Conforms to policy governing patient abuse, patient rights, and
confidentiality of information.
- Assures that the instructions and counseling given to patient
are consistent with that of the responsible Medical
practitioner.
- Participates in in-service department and continuing education
program in order to foster self-growth.
- Participates in departmental and interdisciplinary conferences
pertaining to policies and procedures affecting nursing
practice.
- Participates in performance assessment and improvement
activities on an ongoing basis and assist with data
collection.
- Maintains professional competence through participation in
continuing education, Inservice, staff development and
self-directiveness in his/her own education process.
- Assist in coordinating and discharge planning monitoring
activity for clinical services. Assists in providing ongoing
technical assistance to interdisciplinary patient care team.
- Assist in planning, directing and coordinating patient care;
assume responsibility for the care, comfort and cleanliness of
patients.
- Able to assume responsibility for the acutely/critically ill
patients.
- Assessment and completion of weekly patient classification and
pressure ulcer report.
- Able to assume responsibility for assisting in the supervision
of non-professional assistants and orientation of new staff.
- Function as a nurse in charge of the unit as districts
nursing/modified primary nursing.
- Accompany patients to MRI on Ambulette/ Ambulance when
necessary.
- Participates in Clinical Pathways, initiating and completing
nursing flow sheet, e.g., Fall Precaution Flow Sheet, Restraint
Flow Sheet, l&O Sheet, Graphic Sheet, and Intermediate Flow
sheet if applicable.
- Conforms to policies governing uniform codes, attendance and
punctuality.
- Demonstrate sensitivity to diversity in workforce.
- Ensures a safe and secure environment free of hazards to reduce
the risk of injury to all customers. (Patients, visitors, and
staff).
- Conducts risk assessments to reduce adverse events.
- Ensures that safety management issues are appropriately
addressed.
- Assess and monitors the patient on telemetry. Obtains rhythm
strip. Notifies MD of changes.
- Provides care of patients but not limited to Respirator,
Cardiac Neurological and GI Renal HIV.
- Start peripheral venous access and maintain intravenous therapy
and site care.
- Manages the patient with the following:Triple lumen
catheterPICC line etc.
- Provides care for patients with chest tube drainage
system.
- Airway management by demonstrating ability to care for patient
on mechanical ventilator and tracheotomy.
- Performs tracheotomy care.
- Performs suctioning and ambu-bagging to maintain airway
patency.
- Prepares patient physically and psychologically for
surgery.
- Monitors and provides care post-surgery/procedure.
- Performs wound care and document status of wound. Explains,
involves and instructs patients/families concerning the therapeutic
regimen and environment of care.
- Effectively utilizes the electronic as well as the paper
medical record.

- A valid New York State license and current registration to
practice as a Registered Professional Nurse in New York State;
and
- Holds, or obtains through facility orientation, a valid and
current certification in Basic Life Support (BLS) through the
American Heart Association (AHA).Special NoteFor assignments in
Post-Acute Care ONLY, individuals meeting the following criteria
may also be considered for appointment: Graduation from a nursing
education program registered by the New York State Education
Department (NYSED) as licensure qualifying for Registered
Professional Nurse (RN) and has applied for licensure as an RN.
These unlicensed individuals may be employed to practice under the
immediate and personal supervision of a NYS licensed, currently
registered, RN for up to 90 days immediately following graduation
pending receipt of a limited permit, which selected candidates must
apply for and receive within the same 90-day period. Once a limited
permit is received, individuals can practice nursing, under the
immediate and personal supervision of a NYS licensed, currently
registered, RN at a NYC Health + Hospitals health care setting for
up to one (1) year from the date of issuance of the limited permit.
In addition, must hold or obtain through facility orientation, a
valid and current certification in Basic Life Support (BLS) through
