• Prepares patient for examination and/or special procedures (remove and safe guard dentures, etc.) Remain with patient during examination.
• Observes patients' condition, mental state and social behavior and reports observation to Nurse-In-Charge.
• Prepares patients for meal and assists with feedings as necessary. Distributes nutritional supplements to patients and record
• Assists and accompanies patients in admitting, discharges and transfer process, while assuring comfort and safety.
• Assists patients in activities of daily living.
• Transport and accompany patient to and from unit when necessary and when no patient escort is available.
• Position and performs a.m. and p.m. care.
• Participate in pain management by informing the nurse of patients' complaint of pain in a timely manner.
• Monitors patient on suicidal precaution and notify the nurse of observation and patient's behavior.
• Monitors patient on fall precaution and complete Flow sheet as per policy.
• Collects routine specimens, urine, stool, sputum, etc.
• Performs other duties as needed.Minimum Qualifications
  • Graduation from elementary school.
