DescriptionPurpose of Position:Under general supervision, serves as
a licensed, independent practitioner responsible for planning,
coordinating, delivering, and managing comprehensive health care
services in a health care setting. Ensures patient and
family-centered quality care to improve outcomes and patient
experience, in accordance with applicable NYC Health + Hospitals
policies and procedures, state and federal rules and regulations,
and nursing standards of care.Examples Of Typical Tasks
- Provides continuous comprehensive medical and nursing care,
including preventive and supportive health care services to
patients and families.
- Manages health problems and coordinates health care for various
patient populations. Identifies and diagnoses illnesses and
physical conditions, and implements therapeutic and corrective
measures.
- Consistent with the established nursing practice, differentiate
between normal and abnormal findings in the screening process to
detect changes in health status that require physician's
intervention.
- Develops and maintains partnerships with community-based
organizations and makes referrals to consulting physicians and/or
other health care professionals and community agencies, as
necessary.
- Evaluate and interpret patient data, in order to plan and
implement appropriate nursing interventions.
- Consult with physicians and other health care practitioners in
the evaluation of health screening findings to make prospective
decisions about treatment that will best support improved outcomes
and patient satisfaction.
- Formulates differential diagnosis and the plan of care based on
medical history, physical exams, nursing assessment, and other
pertinent information. Identifies health and medical risks and
needs.
- Develop, implement, and evaluate patient plan of care.
Determines effectiveness and modifies as per clinical standards and
practice protocols, where needed, to achieve medical and health
goals.
- Orders, conducts and interprets diagnostic and laboratory
tests. Identifies and may prescribe appropriate pharmacological and
non-pharmacological interventions and therapies.
- Manages a patient session schedule allowing for both scheduled
and non-emergent unscheduled visits; maintains a work schedule that
ensures availability and continuity of care; ensures optimal
availability and utilization of appropriate resources.
- Provide education, guidance, and direction to nursing staff and
other members of the health care team to ensure quality and safe
medical and nursing care is provided.
- Provides education and counseling to patients and families
regarding health status, treatment and progress, medication
management, disease prevention, and health promotion, records, and
maintains a system of relevant documentation.
- Performs other related duties, as directed.Allergy/Immunology
DepartmentWe are seeking a compassionate and skilled Nurse
Practitioner to join our Allergy and Immunology team in New York
City. As a Nurse Practitioner specializing in allergy, you will
play a crucial role in evaluating, diagnosing, and managing
allergic conditions and immunologic disorders in pediatric and
adult patients. You will collaborate closely with allergists,
immunologists, and other healthcare professionals to provide
comprehensive care, promote patient education and self-management,
and optimize health outcomes for individuals with allergies and
related conditions.Essential DutiesConduct thorough assessments of
patients presenting with allergic symptoms and immunologic
disorders, including medical history reviews, physical
examinations, and interpretation of allergy testing
results.Collaborate with allergists and immunologists to develop
individualized treatment plans for patients with allergic rhinitis,
asthma, food allergies, eczema, urticaria, angioedema, and other
allergic conditions, incorporating evidence-based guidelines and
best practices.Perform and interpret allergy testing procedures,
including skin prick tests, patch tests, and specific IgE blood
tests, to identify allergens and sensitivities and guide treatment
decisions.Provide education and counseling to patients and their
families on allergy triggers, avoidance measures, medication
management, immunotherapy options, and emergency response plans for
anaphylaxis, to empower patients to manage their condition
effectively.Prescribe and administer immunotherapy (allergy shots)
and biologic therapies as indicated for allergic diseases and
asthma, monitoring treatment response and adverse reactions
closely.Manage acute allergic reactions and exacerbations of
allergic conditions, providing prompt evaluation, treatment, and
follow-up care in outpatient and urgent care settings.Coordinate
care across healthcare settings and specialties, collaborating with
primary care providers, specialists, and ancillary services to
address patients' holistic needs and optimize outcomes.Participate
in multidisciplinary team meetings, case conferences, and quality
improvement initiatives to enhance the delivery of patient-centered
care and promote continuous improvement in clinical
practice.Maintain accurate and up-to-date medical records,
documenting assessments, interventions, and patient communications
in accordance with regulatory and organizational standards.Adhere
to ethical and legal standards of nursing practice, respecting
patients' rights, confidentiality, and autonomy, and advocating for
their best interests.Minimum Qualifications
- A valid New York State license and current registration to
practice as a Registered Professional Nurse issued by the New York
State Department of Education (NYSED); and
- Current certificate as a Nurse Practitioner issued by the
NYSED; and
- Holds, or obtains through facility orientation, a valid and
current certification in Basic Life Support (BLS) through the
