Job Description:
AccentCare is seeking a dedicated and compassionate Registered
Nurse (RN) to join our Hospice team. In this role, you will be at
the forefront of delivering personalized, high-quality care and
support to patients in their homes during their critical phases of
end-of-life care. This position involves working closely with
patients and their families to provide emotional support and
medical care designed to manage symptoms and provide comfort. You
will be a key part of a multi-disciplinary team that also includes
physicians, social workers, and other healthcare professionals to
ensure a comprehensive, holistic approach. Assess, plan, and
implement nursing care plans for hospice patients Monitor patient
conditions and report changes to the healthcare team Educate
patients and their families on end-of-life care and symptom
management Administer medications and treatments as prescribed by
the attending physician Coordinate with a multidisciplinary team to
ensure comprehensive care Document patient care services by
charting in patient and department records Provide emotional
support to patients and their families Strong clinical skills in
palliative care and symptom management Excellent interpersonal and
communication skills Ability to work effectively as part of a
multidisciplinary team Strong organizational and time-management
skills Compassion and a deep commitment to patient care Medical
Insurance Dental Insurance Vision Insurance 401k Valid RN license
CPR certified Certification in hospice and palliative care nursing
preferred (CHPN)
