Job Description Job Description Physician Practice Enhancement is seeking an experienced Mental Health Therapist LCSW or LPC for Partnership Health in Franklin Township/Somerset. We are an outpatient office open only to the employees of the School Districts of Franklin Township. Schedule will be 3 days a week 16-24 hours per week This position requires an active LCSW or LPC license provided by the Office of NJ Consumer Affairs to perform therapy in the state of New Jersey. Responsibilities included but are not limited to: Communicate effectively with patient, staff and physician/coordinate care according to the patient's needs. Facilitate individual and/or group counseling services. Develop appropriate treatment goals with clients and work collaboratively to accomplish treatment goals. Document the assessment of patient progress and interventions provided in the medical record (EMR) on an ongoing basis. Recognize, respond to, and assist in psychiatric crisis situations and provide the intervention(s) necessary, including referral to other resources. Counselor Requirements and Qualifications: Clinical experience as a self-directed/motivated mental health professional. Possesses strong written and spoken communication skills, including excellent interpersonal skills with clients, colleagues, and community resources. A team player and collaborate with others under the direction of the Medical Director(s). Excellent time management skills; organized and able to prioritize. Knowledge of Microsoft Office Suite (Word, PowerPoint, and Excel), as well as other software programs such as Electronic Medical Records is a plus. Education: A Master’s degree is required. .
