Social Worker- Home Hospice Per Diem - as needed Day Shift (8am - 5pm) This role works in collaboration with the Interdisciplinary Team to provide social work interventions, education, counseling and support services to address the identified psychosocial needs of patients and caregivers. MSW required, LSW or LCSW must be obtained within two years from date of hire. This position covers Lancaster, York and surrounding counties. Must be able to complete 4 - 5 weeks of a FT, day shift orientation.
